Qualifications:
OBJECTIVE/PURPOSE
Under the direction of the Developmental Services (DS) Director, the Service Coordination (SC)
Program Manager oversees the coordination of services for individuals with intellectual disabilities. The
Service Coordination Program Manager is responsible in ensuring that the Service Coordination
Department adheres to the policies and procedures set forth by the State of Vermont, Department of Aging and Independent Living (DAIL).
MAJOR RESPONSIBILITIES
- PROGRAM DEVELOPMENT AND SUPERVISION
- Oversee Service Coordination and ensure that the department is keeping high quality of services to cases, promoting, and providing the support needed for all DS clients.
- Stay abreast of rules and regulatory changes to provide guidance to the department.
- Maintain a positive and dynamic relationship with provider agencies, clients, guardians, and other stakeholders.
- Provide technical support to Service Coordinators and assist in resolving issues that a Service Coordinator might have.
- Provide ongoing training and coaching to Service Coordinators.
- Track, review and monitor compliance of the Service Coordinators to Documentation Timelines.
- Monitor the department's productivity and ensure timely submission of notes; read notes and provide feedback to Service Coordinators.
- Run reports in the database & any reporting tool to monitor the department.
- Audit closing and transferring cases on a weekly basis and close the case in database as needed.
- Review and analyze data that is related to dashboards and prepare for leadership agenda.
- Analyze reports and present trends that can affect the departments performance.
- Work collaboratively with other DS programs and supports in addition to the DS clinical team to ensure good and open communication.
- Provide and document supervisions with Service Coordinators. Ensure SCs complete required trainings in program models and database systems. Provide and support ongoing skills training and supervision to deliver standards of care that meet the expectations of DAIL. Approve timecards, engage in performance management and corrective action, hiring and selection of staff.
- Participate in ongoing internal and external trainings aimed at enhancing program standards, practice, and client outcomes.
- Adheres to all state regulations.
- NETWORKING AND CONSULTATION / EDUCATION
- Serves on agency committees as assigned and appropriate.
- As key members of the Leadership Team, the Service Coordination Manager should demonstrate passion and enthusiasm for supervision, and embrace concepts of teamwork, collaboration, and quality improvement.
- Participate in ongoing internal and external trainings aimed at enhancing program standards, practice, and client outcomes.
- Maximizes the agency/division's reputation and cultivate support for agency services.
- Effectively represents UCS to the general public and to referral sources.
- Function as a representative of the agency as assigned.
- INFORMATION MANAGEMENT, RECORD KEEPING, AND FISCAL MONITORING
- Completes various clinical and administrative forms for billing, management information, and clinical purposes.
- Prepares reports, as requested.
- Reviews management and outcome reports completed by managers.
- Reviews waiver budgets as appropriate and advise the Division Director of necessary changes.

EDUCATIONAL REQUIREMENTS AND EXPERIENCE
Bachelor's degree with relevant Service Coordination experience. (QDDP) Qualified Developmental Disabilities Professional strongly desired. Familiarity with the Vermont System of Care Plan.
Additionally: Windows skills, typing, navigation, and e-mail usage skills are required. Strong interpersonal, communication, and writing skills are desired.
