Summary

This position is located in the Outpatient Clinic at the Crow/Northern Cheyenne Hospital located in Crow Agency, MT. The purpose of this position is to provide nursing care coordination to a primary care panel of patients. The goal of care coordination is to improve the quality of care to the empanel patient.

A housing subsidy of up to $699 per pay period without out dependents or $830 with dependents may be available.

Relocation, Recruitment and Retention Incentives may be available.

Duties

Ensure Patients are routed to appropriate service, assisting patient with health literacy and serving as their advocate.
Responsible for coordinating care with other departments and programs developing and implementing patient care and education activities necessary to reach positive patient outcomes.
Follow up with empaneled patients to ensure appointments are kept, improve adequacy of young adult and adult patient health and tracking patients seen by various Specialty providers
Plans and participates in Programs for Primary Care Medical Home (PCMH) Management and Primary Prevention Services and tracks patient data outcomes.

Education

Degree from Foreign Nursing School: Official certification from the is required for individuals who graduated from foreign nursing schools.

Basic Requirements for the GS-5 (or equivalent) Grade Level

To qualify at the GS-5 (or equivalent) grade level, applicants must meet one of the following qualification requirements:

Possession of a bachelor's degree in Nursing with no experience; or
Possession of a diploma or associate degree in professional nursing and one full year of professional nursing experience.

Basic Requirements for the GS-7 (or equivalent) Grade Level

In addition to the above Basic Requirements, applicants must meet one of the following requirements:

Possession of a bachelor's degree in Nursing and eligibility under the
Possession of a bachelor's degree in Nursing and at least one full year of professional nursing experience (equivalent to the GS-5 grade level);
Possession of a diploma or associate degree in professional nursing and two full years of professional nursing experience; or
Completion of at least one full year of graduate education in a professional nursing educational program.

Basic Requirements for the GS-9 (or equivalent) Grade Level

In addition to the above requirements, applicants must meet one of the following requirements:

Possession of a master's degree in professional nursing; or
At least one full year of professional nursing experience (equivalent to the GS-7 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program.

MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level (GS-9) in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Promote self-management of chronic disease through helping patients to identify barriers to making healthful choices. Help patient identify barriers to adhere to the prescribed regimen, providing coaching and guidance on positive health behaviors and providing chronic disease management. Assess, plan, implement and evaluate the care of patients with medical, surgical, pediatric, newborn, obstetric, ambulatory and emergency conditions in the primary care clinic. Provide chart review to identify patients at risk for increased morbidity and mortality and recommended interventions. Provide patient and family education to assist in
understanding and accept implications of care, diagnosis and treatment.
